Despite of extensive world-wide efforts the results in the treatment of hypertensive patients are unsatisfactory. A too schematic use of the proclaimed antihypertensive step-therapy may be one reason for this. Within the scope of this step treatment the modern therapy of hypertension should be arranged individually to every patient. The aim is a controlled, slow reduction of the high blood pressure into the normal range in accordance with a preserve or a improve of the quality of live and the secondary prophylaxis of cardiovasculary complications or diseases. The relevant criterias for this are discuss as well as the most important side effects of various antihypertensive substances with a special regard to the lipid metabolism. Finally some proposals relating to the therapy of hypertensives with coincident diseases are submitted.
