Anesthesia for a patient with myotonic dystrophy

57 year-old female patient with myotonic dystrophy underwent a left mastectomy for breast tumor. Since injectable dantrolene was not available, dantrolene was given through a stomach tube to prevent malignant hyperthermia. Only 25mg of intramuscular pethidine as a premedicant for her with 42 kg of body weight caused respiratory depression. A modified neuroleptanesthesia with induced hypotension was car・ied out without event. At the end of anesthesia, naloxon was useful to differentiate pro. longed respiratory depression due to narcotics from the one due to effect of muscle relaxant. Since patients with myotonic dystrophy are very vulnerable to most anesthetic drugs, as well as muscle relaxants, they should be used with a specific caution.
