41-year-old woman complained of continual discomfort after acutely twisting the left ankle 3 weeks before the onset of symptoms. Of note was a history of complete union of a distal tibial fracture sustained 1 year previously. Physical examination revealed a bony hard mass in the right Achilles tendon. Radiographs of the left ankle revealed extensive osssification involving the Achilles tendon (Fig. 1). Within the mid aspect of the distal ossific mass was an indistinct horizontal line representing a healing fracture.
