A 25-year-old primigravida presented with painless vaginal bleeding 8 weeks after her last menstrual period. She had been on Clomid for the preceding month. An ultrasound examination demonstrated two intrauterine gestational sacs with a fetal pole identified in the larger sac, and a 5 cm retention cyst was present in the right ovary (Fig. 1). A threatened miscarriage was diagnosed. A further ultrasound examination 1 week later showed a healthy continuing pregnancy of 9 weeks and a collapsed gestational sac.
