Lyme arthritis presenting as adult onset Still's disease

Lyme disease and adult onset Still's disease share several clinical characteristics such as fever, lymphosplenomegaly, skin involvement and arthritis. An infectious agent has been suspected, at least as a triggering factor, in adult onset Still's disease. A case report is given of a 36-year-old woman from Italy who was eventually diagnosed as having atypical Lyme disease but who had originally been diagnosed as having adult onset Still's disease. Borrelia burgdorferi infection must be suspected in cases of arthritis suggestive of adult Still's disease.
