Data were presented on blood flow recovery in the lower limbs in extensive occlusive lesions of the aorta and arteries of the limbs, in their combined lesion in particular. Aortofemoral shunting was performed on 226 patients with obliterating atherosclerosis and 9 patients with obliterating endarteritis. Restoration surgery is needed to eliminate lower limb ischemia. The type of surgery depends on the type of lesion of major arteries. Aortofemoral shunting is sufficient for the elimination of limb ischemia in isolated lesions of the aorta and iliac arteries. Aortofemoral shunting combined with the plasty of the deep femoral artery or aortofemoral, femoral-popliteal (or tibial) shunting are indicated in combined lesions, lesion of the deep femoral artery in the presence of extensive ulcerative-necrotic changes of the lower limbs in particular.
