Antiarrhythmic and Antifibrillary Antazoline: A Clinical Study

A-HCl P.O. and A-PO4 IV proved in this series to be about as effective an antiarrhythmic and antifibrillary drug as quinidine and procaine amide. Small inadequate doses were used at first. Maintenance dosage of antazoline spaced recurrences often for several months. Intermittent dosage was quite successful. Antazoline suppressed and converted cardiac mechanism disorders. In this series the spectacular interruption of the tachycardias in patients with the WPW syndrome and the normalization of PR, restoration of Q, and clearance of delta were most interesting. Recently paroxysmal atrial fibrillation was converted to SA rhythm in 15 to 18 patients. One with atrial flutter of some months duration was changed to 1:1 by A-PO4, and the next day by Cedilanid to atrial fibrillation, and then by A-PO4 to SA rhythm with embolism. Our rate of conversions of atrial fibrillation and of atrial flutter was higher in our series than in any other series. Our results in patients with myocar-dial infarction and scars warrant further careful study. Paroxysmal atrial flutter was interrupted in 6 patients and slowed only in two patients. Increased ventricular rate was noted in only two patients after antazoline was administered intravenously. Aged patients and those with extensive coronary artery disease with extensive infarctions or scars and extreme congestive heart failure are poor risks for any drug therapy because of the already depressed state of the myocardium. Antazoline as well as quinidine, procaine amide and digitalis depresses the myocardium but possibly to a lesser degree. Myo-cardial infarctions, 10 acute and 10 healed, complicated by mechanism disorders, resulted in death in 9 and survival of from 1 to 15 months in 11 patients. Antazoline cleared the disorders in the patients and probably reduced the mortaility somewhat. Antazoline in our experience has proved to be a good antiarrhythmic and antifibrillary drug.
