The case of a 10-year-old girl from Madhya Pradesh, India, who had pulmonary dirofilariasis and a portal cavernoma is reported. Following surgery to relieve portal hypertension (diagnosed after she presented with haematemesis), she developed respiratory distress and died a day later. At autopsy, the lungs were markedly congested and firm. Immature Dirofilaria immitis were identified histologically, in the right lower lobe of the lung. No parasites were detected in the portal vessels or vena cava.
