Coronarography in patients with hyperlipoproteinemia

Coronarographic investigation was performed in 31 patients followed in the centre for hyperlipoproteinaemias. In the majority of cases ischemic heart disease was the indication of examination. Of the particular hyperlipoproteinaemia types the largest group among coronarographed patients were those suffering from hyperlipoproteinaemia type IIa (16) and type IIb (10). Pathological finding (i. e. presence of stenosis exceeding 50% of vessel lumen) was proved in 24 patients. The most frequently affected artery was ramus interventricularis anterior (16 .times.). Stenosis on the right coronary artery was found in 14 patients. Affection of more arteries simultaneously was frequent. Extensive diffuse changes covering the whole coronary bed as far as periphery were discovered in 3 patients. Pathological ventriculographic finding was scored in nearly 50% of the patients. Cardiosurgery was indicated in 13 patients. Mutual relations of plasmatic lipid, lipoprotein and apolipoprotein levels with regards to the degree of coronary atherosclerosis are discussed as well as possibilities of retardation concerning atherosclerotic process progression by means of influencing lipid levels. At the time when a new concept dealing with hyperlipoproteinaemic patients care is coming into existence it is necessary to stress the complex approach towards these patients.
