Autoimmune hemolytic anemia caused by anti-D antibody and a lymphoproliferative process in a patient with a kidney transplant immunosuppressed with cyclosporine

An O Rho(D)-positive patient, the recipient of an identical ABO renal graft from a Rho(D)-negative donor and on immunosuppression regimen with cyclosporin, developed a hemolytic crisis two weeks after transplantation; an autoantibody with anti-D specificity was detected. It is postulated that traveler B-lymphocytes, that are carried by the donated organ, are responsible for the production of these antibodies. Four months after transplantation a lymphoproliferative disorder of B-cell lineage was diagnosed; it was associated with Epstein-Barr virus infection. Complete remission was achieved when cyclosporin therapy was withdrawn; polychemotherapy and radiotherapy were not needed.
