Clinical experience of Bentall's operation

From March 1981 to April 1987, a total of 21 patients were operated on for ascending aortopathy associated with aortic insufficiency. The ages ranged from 17 to 61 years with a mean of 42.4. Seventeen (81%) were males and four (19%) females. Of these 21 patients, 17 had anuloaortic ectasia (9 met the criteria of the Marfan syndrome), and 4 had a dissecting aortic aneurysm (DeBakey type I or II). The surgical procedure in all cases consisted of total replacement of the ascending aorta with a tube graft containing a prosthetic aortic valve and reimplantation of the coronary arteries to the tube graft. Six patients underwent an emergency operation, four for acute dissections, one for a ruptured aneurysm during cardiac catheterization, and one for severe congestive heart failure due to aortic insufficiency. Four of these 6 patients died during hospitalization. The causes of death included low cardiac output in 2, intractable ventricular tachycardia in 1, and bleeding in 1. Fifteen patients, 11 of whom had congestive heart failure, underwent an elective operation. One of them died of low cardiac output after operation, and the other 14 survived and were discharged from the hospital in good condition. The follow-up period ranged from 4 to 77 months, with a mean of 34.8 months. One late death occurred 2 years after operation. The 15 surviving patients all have improved clincal status and exercise tolerance.
