Clinico-biochemical characteristics of hepatic insufficiency in peritonitis

Study of the incidence of hepatic insufficiency and the main factors promoting its occurrence has shown that it is most common in appendicitis and pancreatitis with a tendency of peritonitis development. The delayed surgical treatment and gross hemodynamic abnormalities, especially in elderly and old patients, contribute to hepatic insufficiency in peritonitis with attendant endotoxicosis and increased peptidemia predominantly at the expense of accumulation of fractions containing aromatic amino acids. Hemosorption included in combined treatment at the early stages of the condition can prevent the development of hepatic insufficiency and improve the results of treatment. It has been postulated that products of protein degradation are responsible for the mechanisms that trigger functional hepatocyte impairment.
