The effect of curantyl (dipiridamole) on the E2 and F2.alpha. prostaglandin (PG) excretion was studied in 18 patients with chronic glomerulonephritis and in 12 of them on renal blood flow. Curantyl (20 mg) was injected intravenously. PG excretion in the urine was studied 1.5 hours before and after administration. Kidney scintigraphy with a radiopharmaceutical was employed to study renal blood flow before and on the 1st-2nd minute after curantyl was administered. Changes in the renal-aortic index that reflected relative changes in the perfusion rate of renal blood flow under the influence of curantyl were calculated. PG excretion was increased in 15 of the 18 patients by 868 .+-. 17.6 per cent, mainly at the expense of PGE2 (133.2 .+-. 33.1 percent) and renal blood flow was increased in 11 of the 12 patients, as was indicated by an increase in the renal-aortic index by 32.1 .+-. 7.7 percent. Thus, curantyl is capable of enhancing renal PG synthesis and improving hemodynamics. This is additional pathophysiological substantiation for using curantyl in patients with chronic glomerulonephritis.
