Small cell carcinoma of the uterine cervix: pathologic analysis of 9 cases

Nine cases of primary small cell (oat cell) carcinoma of the uterine cervix are reported. Histologically, they are purely small cell carcinoma in 4 cases, mixed with squamous cell carcinoma in 3 cases and mixed with adenosquamous cell carcinoma in 2 cases. Regional lymph node metastasis occurred in 6 cases: 5 cases revealed metastatic small cell carcinomaonly and 1 showed both small cell carcinoma and adenosquamous cell carcinoma. Immunohistochemical studies demonstrated neuron specific enolase (NSE) in all with one exception, which revealed membrane-bound, dense-core neurosecretory granules on electron microscopic study. Epithelial membrane antigen (EMA), cytokeratin (AE1/AE3), and carcinoembryonic antigen (CEA) were demonstrated in the small cell component of 5 cases. The stainings for calcitonin, somatostatin, adreno-corticotrophic hormone, and Leu7 (a myelin-associated protein in neuroectodermal tissue) were all negative. Clinically, these tumors behaved very aggressively; 7 cases (78%) died within 2 years. With regards to the frequent occurrence of mixed histologic patterns, the histogenesis of the tripartite differentiation in one tumor is discussed.
