Variants of ischemic heart disese (IHD) in diabetes mellitus patients have been discussed. The course of IHD and myocardial infarction in diabetes mellitus and in its absence is compared in two groups of patients (a total of 312 subjects). In addition to typical angina pectoris that accompanies diabetes mellitus, the pain-free forms of 1HD are encountered two-four times more often; myocardial infarctin more frequently runs a couse without typical status anginosus, and the early and long-term prognosis of myocardial infarction is more serious. A postmortem study demonstrated in most cases stenosing coronary cardiosclerosis but no correlation was revealed between the severity of angina pectoris and the degree of stenosis. The pathogenic mechanisms that can be responsible for pain-free IHD course in diabetes mellitus and the methods of diagnosis of IHD with its pain-free course are discussed.
