The author reviews recent changes in South African health care. These have perpetuated the fragmentary and wasteful system with multiple parallel sectors that are poorly co-ordinated. Increasingly, segregation is taking place on economic rather than racial grounds with an expanding private sector providing high quality and sophisticated care. Population growth and urban migration, combined with the effects of poverty, have overwhelmed the public sector, with patients being discharged from hospital prematurely. The situation is exacerbated further by the movement of trained staff from the public to the private sector.M. McKee.
