Exercise hemodynamic evaluation of postinfarction left ventricular aneurysmectomy

To assess a left ventricular functional reserve, 7 patients underwent hemodynamic studies at rest and during exercise following left ventricular aneurysmectomy (LVA). 1) Hemodynamics at rest: The left ventricular end-diastolic volume reduced significantly following LVA (p<0.05). The ejection fraction increased from 0.27 to 0.46 after surgery (p<0.01), and the cardiac index recovered to normal range postoperatively (p<0.05). 2) Hemodynamics during exercise: Preoperatively the mean SVI did not show any significant increase, however after surgery, it did increase with exercise (p < 0.05). The stroke work index increased significantly both at rest (p < 0.05) and during exercise (p < 0.05), although the LVEDP did not fall significantly either at rest or exercise after surgery. Considering these hemodynamic changes following LVA, improvement of preload reserve occurred and there by a favorable shift of Farnk-Starling curve of the new left ventricle resulted.
