Investigation on influence factors of pulmonary hypertension in chronic cor pulmonale

Observations were made on 289 cases of chronic cor pulmonale in exacerbation as to what influences of hypoxemia, hypercapnia, high pack cell volume (PCV) and blod viscosity (BV) would be exerted on their pulmonary hypertension (PHT). The results showed that the mean pulmonary artery pressure (.hivin.pPA) was markedly increased in 84 cases (29.1%), moderately and mildly increased in 67 cases (23.2%) and 77 cases (26.6%) respectively. In 61 cases (21.1%), however, the .hivin.pPA was within normal limits. The correlation of .hivin.pPA with PaO2 value was negative, but the correlation of pPA with PaCO2, PCV and BV values was positive. Such facts may indicate that the primary etiological factors of PHT are hypoxemia and hypercapnia, while the high PCV and BV induced by hypoxia are the factors that further aggravate the PHT. Experiments have indicated that hypoxemia, hypercapnia, high BV, etc. are the reversible or functional factors of PHT in patients with chronic cor pulmonale in exacerbation. Therefore, in addition to the basic treatment of anti-respiratory infection and improvement of ventilation, using the long-term oxygen therapy, erythropheresis and calcium antagonists for the prevention and reversal of hypoxic PHT in chronic cor pulmonale is very important in the management of such patients.
