Difficulties in diagnosis of early stages of carious lesions were demonstrated at 85 premolar teeth with discoloured fissures. Probing with dental explorers and Miller's probes seems to be uncertainly and may cause an iatrogenic damage. Comparison of diagnoses before and after cavity preparation showed a progression of the carious process into the dentin more frequently than expected. Invasive treatment was justified in all cases.
