The case of a woman aged 28 years with a large cyst in the right suprarenal gland is presented. The diagnosis was not established preoperatively. The diagnosis may be difficult to establish because ultrasonography, CT and MR scanning frequently do not provide conclusive data about the origin of the cyst. Endocrinological assessment, ultrasonography, CT scanning and puncture of the cyst with cytological examination and adrenal hormone analysis of the cystic fluid are recommended. Operation is required when the cyst produces symptoms or if malignancy is suspected.
