Coronarography during the acute phase of myocardial infarction

Left ventriculography and coronary arteriography have been performed in 119 patients below 70 years of age, with their first myocardial infarction. All investigations were performed within 4 hours after the onset of infarction. Coronary arteriography revealed coronary obstruction in all cases, and a complete occlusion in 81%. By means of intracoronary interventions (intracoronary nitroglycerin, intracoronary streptokinase) anterograde flow was restored in 66%. Coronary vasospasm occurred in 10%, coronary thrombosis in 68% as reflected by changes after selective drug treatment. A significant residual stenosis was revealed in 80% of the cases after successful restoration of blood flow. After successful recanalization reocclusion is frequent. The high incidence of multi-vessel disease, and that of reocclusion may variant to consider the revascularization of patients after a first myocardial infarction.
