Influence of hyperlipidemia on late patency of aortocoronary bypass grafts

Influence of hyperlipidemia (HL) on late patency of aortocoronary bypass grafts was evaluated in 173 saphenous vein grafts (SVCs). HL was defined as total cholesterol (TC) level of 250 mg/dl or more and/or triglyceride (TG) level of 150 mg/dl or more. Regarding to the type of HL, SVGs were divided into three groups as follows: Group A: Non HL (83 SVGs) Group B: preoperative HL and postoperative non HL (24 SVGs) Group C: pre and postoperative HL (66 SVGs) Mean blood flow of grafts, mean follow-up duration, mean blood lipid level and the late patency rate were compared among the three groups. The results were as follows. 1. There was no difference among three groups in regard to mean flow of grafts, mean follow-up duration. 2. Mean preoperative TC level in Group C was significantly higher than in Group B (p < 0.05). 3. Late patency rate of grafts was 91.6% in Group A, 95.8% in Group B and 77.3% in Group C, respectively. Late patency rate in Group C was significantly lower than those in Group A and B (p < 0.05). In Group C, there were two patients who had undergone re-AC bypass grafting for late graft occlusion. In conclusion, the late patency rate of sphenous vein grafts(s) was generally unsatisfactory in those whom HL persisted, but it was improved in those whom HL was adequately treated.
