Endoscopic extraction of ureteral calculi by the combined use of the Zeiss loop and Fogarty catheter

We describe a method for endoscopic removal of ureteral stones using the Zeiss loop to grasp the stone and the Fogarty balloon catheter to dilate the ureter to facilitate stone extraction after it has been grasped. Thereafter both catheters simultaneously pull the stone down into the bladder. Placement of the balloon catheter just below the calculus distends the ureteral wall and helps dislodge the calculus. Furthermore, this maneuver facilitates stone extraction and prevents lesions to the ureter during removal thereby achieving a higher success rate and reducing risk. The Zeiss basket and the Fogarty balloon catheter used in combination can extend the indication for endoscopic removal of calculi with catheters to include those located higher up and larger calculi. We present 9 patients in whom the combined maneuver was performed. A success rate of 77% was achieved. In 3 cases the stone was at the level of the lumbar ureter. The remaining 6 patients had undergone previous attempts of blind endoscopic stone extraction without success. We believe that this simple maneuver has a modest place in the therapeutic armamentarium currently available for endoscopic resolution of ureteral lithiasis. Moreover the low cost of this maneuver makes it widely available.
