ECG diagnosis of myocardial infarction in patients with pacemaker is very difficult to make and it must be based on clinical, laboratory and instrumental data. The present study points to ECG changes on which an acute inferior myocardial infarction can be diagnosed even with a pacemaker in place, particularly when the stimulation is bipolar (temporary). In the case of infarctions located elsewhere the ECG pattern of QRS complexes at simultaneous stimulation is so much altered that ECG diagnosis is a problem. For this reason, spontaneous QRS complexes must be elicited either using a drug or by repeated short-term inhibition if the VVI pacemaker. The mentioned conclusions are demonstrated on two clinical observations.
