Indication of high frequency jet ventilation for respiratory failure

To determine the indication of high frequency jet ventilation (HFJV) for repiratory failure, we have done experimental and clinical studies. In experimental study, at first we compared the effects of HFJV with CPPV at the same PEEP level. And secondly we attempted a new technique of HFJV, so-called combined HFV (CHFV) in dogs with oleic-acid induced pulmonary edema. CHFV is the combinatin of HFPPV (1 Hz) and HFO (10, 20 Hz): HFPPV is used to eliminate CO2 and HFO provide oxygenation. The results were as follows: HFJV, even CHFV didn't improve oxygenation rather than CPPV, but reduced the airway pressure evidently, especially in CHFV. And HFJV eliminated CO2 effectively at low frequencies (1 .apprx. 3 Hz). In clinical study, we used HFJV for respiratory failure with high-airway pressure, and investigated the availability of HFJV. The result revealed that HFJV successfully treated the ventilation of the states of bronchospasm, wet case, atelectasis and air leakage. Therefore, we concluded that HFJV should be used for the purposes of reduction in the airway pressure, elimination of CO2 at low frequencies, excretion of sputum (physio-therapeutic effect), and further, ventilation in the cases of the air leakage.
