Computed tomographic (CT) scans were obtained from 5 patients with herpes simplex encephalitis. A characteristic but not pathognomonic pattern, which consisted of low attenuation in one or both temporal lobes with extension into the frontal lobe(s), was observed. This change needed at least 4 days after onset of neurological symptoms to be detected in CT. Compared with electroencephalography, CT was less sensitive in the early detection of the illness, but the extent of involvement by the inflammatory process could be appreciated on the follow-up CT studies.
