Significance of liver cell dysplasia in cirrhosis & hepatocellular carcinoma

A comparison of liver cell dysplasia (LCD) in cirrhotic livers and hepatocelluar carcinoma, was made in material collected from Senegal, India and USA which are countries with high, intermediate and low incidence rates for HCC respectively. Control livers without primary liver disease showed no LCD. Hepatocellular carcinoma, on the other hand, irrespective of the geographic area, was associated with LCD in more than 80 per cent instances. Cirrhotic livers from Senegal and India showed LCD in approximately 70 per cent while those from USA showed in 43 per cent. The positivity rates for LCD was related to that for hepatitis B virus surface antigen (HBsAg) only in cirrhosis but not in hepatocellular carcinoma. LCD appears to be strongly associated with occurrence of HCC irrespective of the involvement of hepatitis B virus (HBV) in the carcinogenesis. The frequent LCD positivity in cirrhosis from areas endemic for HBV infection suggests a susceptibility of these livers to develop hepatocellular carcinoma. LCD needs to be differentiated from polyploidy in the liver, which is age-related.
