Between January 1994 and July 1995, 5221 patients under the age of 15 years were seen at an emergency unit in Kinshasa, Zaire. Examination of thick blood films showed that 408 (59%) of 696 with a PCV <=20% were positive for Plasmodium, compared with 2233 (49%) of 4525 with a PCV >20%. Packed red-blood cell transfusion was given to those patients with a PCV of 20% or less. All blood was tested for HIV-1 by ELISA and Western blotting. The risk of HIV-1 transmission in such blood transfusions are discussed.
