A lingual arch for anchorage of the upper molars

We collected two groups of patients with malocclusions of Class II, division 1 or Class I with protrusion of the upper incisors and overjet of 7 mm and more. The patients were treated with fixed orthodontic appliances of the light-wire edgewise type. They had teeth extracted in the region of the upper premolars. In the first group of 22 patients the lingual arch soldered to the bands of the upper molars was used for anchorage. In case the anchorage required it, extraoral pull was added. In the second group of 35 patients the upper molars were anchored only with extraoral pull. The mean time of using extraoral pull was shorter in the group of patients with lingual arches and in 31.8% of them extraoral pull was not necessary. The lingual arch prevented mesial rotation of the upper molars, which was 11.degree. on either side in the group without the lingual arch. We recommend the lingual arch as a basic anchorage when extraction therapy in the upper dental arch is used. The anchorage is then, according to the necessity, supplied with the extraoral pull.
