A new model of heterotopic cardiac transplantation for biventricular bypass in the dogs

A new model by using the dogs which is available in examining the donor heart function after preservation and immunosuppressive administration has been developed. In this model the transplanted heart works as total LV bypass and partial RV bypass. The procedures are as follows: a) D-LA, R-LA side to end anastmosis. b) D-mPA, R-ltPA side to end anastmosis. c) D-desc Ao, R-desc Ao side to end anastmosis. d) resuscitation of donor heart. e) D-IVC, R-IVC side to end anastmosis. f) ligation of R-ascAo. Preliminary experiments demonstrated that 5 out of 7 dogs (Beagle) with the donor heart pretreated with diltiazem survived over 1 week as long as 50 days under minimal immunosuppressive agent. This model has several advantages over various conventional models reported. 1) Intracardiac procedures such as ASD and TR creation and also conduits are unnecessary. 2) It can be performed without extracorporeal circulation and systemic heparinization and leads to low mortality. 3) Transvenous endmyocardial biopsy of both D- and R-RV are possible. 4) Total LV bypass in addition to partial RV bypass enables us to estimate physiologically working donor heart function. In conclusion this experimental model is of such benefits to assess not only the preserved heart but also the effectiveness of immunosuppressive regimen. Without ligation of R-ascAo this model can be of clinical use as a heterotopic transplantation in the left hemithorax.
