Primary hyperparathyroidism with symptoms of osteomalacia

Osteomalacia in cases with primary hypercalcemic hyperparathyroidism may lead to the normalization of calcemia. The present study was aimed at comparing clinical symptoms, calcium and phosphorous metabolism, and PTH level in serum with histomorphometric examination of the normal osseous tissue in patients with possible hyperparathyroidism or osteomalacia. The study involved 6 patients aged between 27 and 72 years: 4 hyper- and 2 normocalcemic. Elevated i-PTH and histomorphometric signs were seen in all patients, including 2 patients with osteomalacia. Hypercalcemia followed low dose of vitamin D2 in 1 normocalcemic patient with coexisting hyperparathyroidism and osteomalacia. Parathyroid adenoma was operated in 6 patients. In case of normocalcemia, hyperparathyroidism and osteomalacia, biopsy of the bones prevents the treatment with large doses of vitamin D and consequently hypercalcemic crisis.
