Ticlopidine is a synthetic molecule widely used in the prevention and treatment of thromboembolic diseases. The most important adverse effects of the drug are haematological in nature, with acute and generally transient agranulocytosis in the foreground. A clinical case of acute bone marrow depression is described with prevalent agranulocytosis observed in the Medical Division of Casalpusterlengo Hospital. In this case the bone marrow aspirate showed a hypercellular marrow with an accumulation of granulocytic precursor cells. The mechanism of action of ticlopidine-associated pancytopenia is discussed.
