Surgical treatment of tetralogy of Fallot with subpulmonary ventricular septal defect

From May, 1965 through December, 1986, 137 of the 1348 tetralogy of Fallot (TOF) patients had undergone intracardiac repair in our hospital were with subpulmonary ventricular septal defect(VSD). The early mortality rate was 3.6%. The incidence of residual VSD was 0.7% and no late death occurred. TOF with subpulmonary VSD was a special form of TOF. If such patients were operated with the "classic" TOF operation method there was a tendency for severe residual pulmonary stenosis. In such patients, a patch-graft enlargement across the pulmonary valve ring was necessary. Subpulmonary VSD was closed with a half circular patch. This report emphasized some features of this subset. The classification, diagnosis and method of corrective operation were also discussed.
