Clinical and pathological study on resection of the thoracic aorta associated with esophagectomy for esophageal cancer--indication and method

Four patients with esophageal cancer underwent esophagectomy and combined resection of the aorta. All of them died of recurrence or complications during hospitalization. In order to improve the results of combined aortectomy in esophageal cancer operation, following considerations should be taken. 1) The case which will have distant organ metastasis during early postoperative period must be excluded from combined aortectomy by careful preoperative examinations. 2) Surgical stress caused by the procedure for aortectomy should be reduced as much as possible. For this purpose, we consider that left thoractomy is preferable. 3) Aortectomy more than 1 cm away from macroscopical cancer invasion is considered to be sufficient as a surgical margin.
