The clinical course and management of 14 patients who developed wound infection after posterior operative exposure of the lumbar spine for laminectomy or spinal fusion are reviewed. The presentation of serious infection was more often subtle than florid. Prompt diagnosis, early systemic antibiotics and thorough local debridement will control infection. Delayed resuture of the wound is recommended and can achieve early soft tissue healing. However, the long-term prognosis remains guarded.
