Influence of pretransplant clinical conditions on the prognosis of patients treated with allogeneic bone marrow transplantation during leukemia relapse

Fourteen patients with acute leukemia were treated with chemotherapy, total body irradiation (TBI) and allogeneic bone marrow transplantation (BMT) to eradicate the refractory leukemic cells during relapse and the prognoses were retrospectively studied in relationship to the pretransplant clinical conditions. The 14 patients were divided into two groups: those with relapsed leukemia but a relatively good clinical condition (Group 1) and those with advanced relapsed leukemia and a poor clinical condition (Group 2). The two groups were compared with respect to the incidence of severe infection, acute graft vs host disease (GVHD), the duration before developing interstitial pneumonitis (IP) and the survival period after BMT. The patients in Group 2 had a greater incidence of fatal bacterial infection and acute GVHD, contracted IP sooner, and had a shorter survival after the BMT, suggesting that the overall pretransplant clinical condition was an important factor in the prognosis of marrow transplanted patients.
