One stage operation of pectus excavatum and annulo aortic ectasia associated with Marfan's syndrome

A case of AAE in a patient with Marfan's syndrome complicated with aortic regurgitation and severe pectus excavatum is reported. In the case of open heart surgery complicated with thoracic deformity, the method of reaching the heart is important. In the usual Bentall's operation, the median sternotomy is applied, but in this case it is difficult to obtain a favorable viewing field due to an extremely deformed sternum and significant displacement of the heart into the left thoracic cavity. We have recently successfully performed Bentall's operation and STO-O simultaneously, with a favorable viewing field, by removing the sternum, en bloc, as a cost-sterno complex (plastron). Our concepts in treating the one stage operation for open heart surgery and thoracic deformity were discussed.
