Histopathological & immunohistochemical changes in placenta due to acute viral hepatitis during pregnancy

Histopathological changes in the placenta following acute viral hepatitis due to hepatitis B virus (HBV) and non A, non B virus (NANB), in the third trimester of pregnancy have been studied. Acute NANB hepatitis was more common during pregnancy with a high incidence of maternal and foetal complications. The placental changes were more pronounced and severe when the mother suffered from an acute fulminant hepatitis, as compared to non-fatal virus hepatitis. The immaturity of trophoblastic villi was an important feature of acute fulminant hepatitis, and was characterized by reduction in the vesiculo-syncitial membranes, thickened basement membrane, stromal fibrosis, hypovascularity, and villous fibrosis or oedema. Immunohistochemical staining demonstrated the deposition of IgG, and IgM along with complement C3 in the areas of fibrinoid necrosis and on the trophoblastic basement membranes in most of the placentae from mothers with acute fulminant NANB hepatitis.
