Decrease in theophylline clearance and, consequently, increase of theophylline serum concentration have been reported after cimetidine was given to asthmatic patients with peptic ulcer disease. Also ranitidine appears to share the ability to alter theophylline clearance, but the interaction is less marked compared to cimetidine. Clinicians should be alert to the significant interaction of these commonly used drugs and theophylline levels should be monitored when cimetidine or ranitidine are given to a patient taking theophylline.
