Counter immunoelectrophoresis (CIEP) for serological diagnosis of typhoid fever

Ultrasonic lysate and veronal-buffer extract antigens (VBE) of Salmonella typhi were used for detection of antibody by counter immunoelectrophoresis (CIEP) in sera of 49 S. typhi culture-positive patients (group I), 50 febrile controls (group II) where causes other than typhoid were established and 96 afebrile healthy controls (group III) with no history of TAB vaccination or typhoid fever in the last one year. Positivity of 87.7 per cent was obtained using the ultrasonic lysate antigen for CIEP, and 69.3 per cent with VBE; and 69.3 per cent by Widal test in the 49 S. typhi culture-positive group. The sensitivity and specificity of CIEP test using ultrasonic lysate was 87.7 and 91 per cent respectively as compared to 69.3 and 87.6 per cent with VBE. The sensitivity and specificity of the Widal test was found to be 69.3 and 89.04 per cent, respectively. All attempts at detection of S. typhi antigen in sera of culture-positive patients were unsuccessful.
