Following a description of the particularities of embryologic development, the premises for the onset of chronic suppuration of the ear, the authors describe the principal anatomopathologic alterations that develop in chronic suppurated otitis media. According to the specific evolution of chronic auricular suppurations, tympanoplasty involves two obligate aspects: preoperative preparation and tympanoplasty. On the basis of their own experience and published data, the authors show that repair of the integrity of the ear drum arrests chronic suppuration and, moreover, may give functional improvement owing to the new tympanum membrane, obtained by simultaneous (when possible) or subsequent columnization. Good results in the tympanoplasties performed by the authors were obtained by autologous columnization with a bone fragment from the mastoid cortex.
