Acute respiratory failure due to massive microembolization of the pulmonary arteries by breast tumor

A 37-year-old woman was admitted because of rapidly progressive respiratory failure, which caused death within 12 hours. She had undergone mastectomy for infiltrating duct carcinoma nine months prior to her last admission. No metastases had ever been demonstrated. Acute respiratory failure was found to be due to almost complete obstruction of the pulmonary micro-arteries and arterioles by tumor emboli embedded in fibrin clots. The origin of the tumor micro-emboli was probably metastases to the liver which had infiltrated the hepatic veins.
