The concentrations of prostatic acid phosphatase (PAP) and carcinoembryonic antigen (CEA) were determined in 126 patients with prostatic carcinoma before treatment and repeatedly during the treatment in 116 patients in this group. The group for comparison comprised 173 patients operated on for prostatic adenoma. A significantly raised initial concentration of PAP was found in 59.8% of cases and CEA in 20.8% of cases of prostatic carcinoma. A significantly higher percentage of patients with positive initial PAP determinations was demonstrated in the group with poor prognosis. In 79.0% of cases a considerable agreement was observed between PAP concentration and the clinical course. In all patients in clinical stages A, B and C in whom skeletal metastases developed during follow-up an increase of alkaline phosphatase activity preceded the appearance of metastases.
