In most cases diseases of the apical periodontium result from disintegration products of the pulp and from bacteria of the root canal. This indicates the need for conservative therapy treatment. Therefore the cleaning of the exact root canal by means of mechanical preparations up to the foramen apicis dentis is necessary and this should be followed by washing with antimicrobial irrigants. Finally the root canal should be filled in with a suitable biocompatible material. The indication for a surgical intervention of the periodontitis apicalis should be limited to teeth where anatomical difficulties prevents adequate treatment of the root canal, secondly to teeth with cysts and furthermore for those cases where conservative therapy is not successfully.
