Exotic infection in the immunocompromised host

A case in a 74-yr-old man receiving corticosteroids and haemodialysis via a left forearm Cimino shunt is reported. The shunt was removed 3 months before admission, but the wound did not heal well, and 1 month prior to admission the forearm became red and swollen, with subsequent vesicle formation. Skin biopsy yielded Cryptococcus neoformans. Blood and cerebrospinal fluid cultures were positive. Treatment with amphotericin B and 5-fluorocytosine was begun and predisone was reduced. On day 21 a tender mass appeared in the right thigh. The abscess was surgically drained. Nocardia asteroides was cultured from the drained fluid. The patient was treated with trimethoprim and sulphamethoxazole.
