Hodgkin's disease primarily arising in the spleen is a very rare entity, which has been observed in only a few instances. Here the authors report a new example occurring in a 60-year-old woman, who showed primary splenic Hodgkin's disease spreading to the liver at the time of diagnosis, in the absence of any other organ involvement. The study is provided with complete follow-up information, autopsy findings and critical review of the literature.
