Clinico-laboratory evaluation of the toxemia syndrome in suppurative meningitis and meningoencephalitis

The severity of the intoxication syndrome was assessed in 73 patients with purulent meningitis and meningoencephalitis who were under observation from the moment of admission to discharge from hospital. Clinical findings, laboratory and biochemical parameters were used to establish the severity of toxicosis. In all cases the blood content of metabolites and mean molecular weight (MMMW) was studied simultaneously. Analysis of clinical and biochemical findings has revealed a distinct correlation between the MMMW values and clinicl symptoms of toxicosis. The MMMW blood content in subjects who died from progressive brain edema varied from 0.870 to 1.44 U. Normalization of the MMMW values occurred when the clinical manifestations of toxicosis regressed, the meningeal syndrome disappeared, and sanation of the cerebrospinal fluid was carried out. The concentration of nitrogenous metabolites during all periods of observation remained unchanged. A correlation was observed between the MMMW and NBT-test values and the level of cation proteins in neutrophils in the acute period of the disease. It is presumed that the use of MMMW parameter together with tests conducted in purulent meningitis and meningoencephalitis will provide more accurate evaluation of the toxicosis syndrome to ellaborate optimal therapeutic measures.
