We report on a case of liver transplantation under Cyclosporin-A (CsA) successfully monitored by fine needle aspiration cytology (FNAB), percutaneous biopsy and urinary exfoliative cytology. We therefore conclude that FNAB constitutes an elective method for the early diagnosis of pathological conditions due to either CsA toxicity or other hepatotoxic drugs, or due to rejection, limiting the classical liver biopsy to conditions in which the FNAB does not allow a diagnostic evaluation.
