The authors present a report dealing with long-term results after implantation of extraanatomical femoro-femoral bypass in 36 patients. The bypass was implanted in patients at high risk with advanced ischaemia of lower extremity cases. In all patients, where the operation could provide a sufficient flow through the bypass the reconstruction remained open for long period of time, if there was not a progress of atherosclerosis in the supplying or out-flowing arteries. The condition for healing gangrenae of lower extremities is an efficient collateral femoro-genicular-crural collateral circulation or a direct revascularization of a. poplitea and a. tibialis. In a long-term obsrevation 70% patients with the bypass open died within five years after the operation. The remaining patients died up to eight years. Most patients with the open femoro-femoral cross bypass died of ischaemic heart disease and its complications.
