Clinical and hemodynamic study in patients with hypotension and shock complicating acute myocardial infarction

Clinical and hemodynamic features of 599 AMI patients admitted from 1980 to 1984 were reviewed, 51 cases (8.5%) were diagnosed with shock syndrome, 68 of the 599 patients (11.4%) died, 24 (35.3%) of the 68 died from shock. The mortality in shock patients was 47%. Invasive hemodynamic monitorings were performed in 8 patients with shock. In the hypotensive group, mean pulmonary wedge pressure (PCWP) was lower than 18 mmHg in 5 cases, higher than 18 mmHg in 3; in the shock group, PCWP was higher than 18 mmHg in 8 cases, lower than 18 mmHg in one. In former group, cardiac index (CI) was more than 2.2 L/min/m2 in 4 cases, less than 2.2 L/min/m2 in one; in the catter, group, CI was less than 2.2 L/min/m2 in all of the 7 cases. The stroke volume index (SVI) and left ventricular stroke work index (SWI) were more than 32 ml/beat/m2 and 32 g .cntdot. m/m2 respectively in all 5 patients of the hypotensive group; but SVI and SWI were less than 26 ml/beat/m2 and 26 g .cntdot. m/m2 respectively in the all 7 patients of the shock group. The systemic vascular resistance (SVR) had no significant difference between two groups. The causes precipitating factors and therapy of hypotension and shock were discussed.
